We kindly request that you complete the below online forms at your earliest convenience, but no later than 48 hours prior to your scheduled consultation.

Filling out these forms in advance enables us to better prepare for your consultation, which allows us to make the most of our time together, focusing on the solutions and guidance you and your dog(s) need.

Cancelation policy: Clients are kindly requested to provide at least 48 hours' notice if they need to cancel or reschedule a dog behaviour consultation appointment. We understand that unforeseen circumstances can arise, and we appreciate your cooperation in giving us sufficient time to adjust our schedule accordingly. Clients who cancel their consultation within less than 48 hours of the scheduled time will be subject to a cancellation fee of $50. We understand that there may be exceptional circumstances that prevent you from attending your scheduled appointment, please contact us as soon as possible to discuss your situation. We will evaluate such cases on an individual basis
I agree to these terms & conditions: As the owner of my dog I am solely responsible for my actions and actions of my dog both during the consultation and in the future. I hereby hold harmless Perfect Paws Behaviour Solutions, their agents, employees, and other individuals involved in providing the service in accordance with this agreement from any and all liability for any loss, damage or injury to persons, animals or property arising from or related to their actions, their pet or their pet’s actions.
